Visiting Nurses Association

The VNA is an affiliate of the Senior Resource Connection and is a Certified Medicare/Medicaid Agency which offers a full range of in-home health care services to patients and their families throughout the Miami Valley and surrounding areas. With its staff of highly trained healthcare professionals and home health aides, the VNA services include:

• Registered Nurse Visits
• Physical Therapy
• Occupational Therapy
• Speech Therapy
• Home Health Aide Visits (Bath Aide)

VNA accepts referrals from a variety of sources including: physicians, healthcare providers, hospitals, social service agencies, churches, family members, friends and neighbors.

How to get started:

• Call the VNA at (937) 222-3381.
• A VNA nurse will then schedule a visit to make an assessment of the patients needs.
• If skilled care is needed, our nurses work with the patient; caregiver and physician to develop a customized Plan of Care, based upon a comprehensive health care assessment.
• Our professional staff will then help the patient and family navigate the social, Medicare and Medicaid system to secure funding to provide the skill care needed.
• In addition to the ongoing medical care that is prescribed, staff work collaboratively with patients and their family’s to insure that any ongoing or anticipated health care daily living needs are addressed.
