Adult Day Service
Hours of Operation - Monday thru Friday 7am to 6pm
Our Adult Day Service provides your loved ones with the opportunities for mental, physical, social and therapeutic activities during the day while still living in a home environment. We offer a secure environment for those individuals who are no longer capable of full-time independent living and require some assistance or supervision.
Our Adult Day facilities allow the caregiver and family members to continue to work while giving the participant and the family member respite, support and education during this difficult time.
Our two facilities offer a cost effective alternative to instructional care with services that include daily nursing and personal care, therapeutic activities, nutritional lunches and snacks, transportation to and from home by certified drivers in ADA approved vehicles.
Our Adult Day Service has two levels of care: levels of care are established by completing a comprehensive nursing assessment.
Enhanced Services
For the individual who needs moderate assistance with daily needs, such as, toileting and mobility. A licensed professional nurse provides regular health assessment and monitoring of health related conditions, including blood pressures and weight monitoring.
Intensive Services
For the individual who needs substantial assistance a licensed professional nurse provides ongoing intensive monitoring with individuals with a chronic medical condition including medication administration, rehabilitative or restorative therapy provided onsite by licensed physical therapists, occupational therapists, or speech therapists. Daily personal care including showers.
How to get started:
- Call the Senior Resource Connection at 937-433-1332.
- We will then schedule a participant assessment by a registered nurse at your home or at the Day Service facility. Transportation can be provided to the assessment if prearranged.
- The program does have a limited amount of care scholarships available. Staff are also well versed on other payment or outside funding opportunities. The program is a passport provider in Montgomery, Greene and Clark, as well as a provider with the VA and levy programs in Montgomery and Greene Counties.
